Session Description

Transitions of care are being pinpointed as one of the weakest links in the US health care system. For patients with kidney diseases, it is well documented that transitions onto dialysis, or in and out of the hospital, are among the most dangerous and costly points in a patient's life. This session examines the roles of nephrologists, primary care providers, and others in the care of kidney patients during transitions. Also, it addresses the policy levers and options to ensure and incentivize the best possible transitions for patients.

Session Objectives

Upon completion of this session, the participant will be able to: 1. explain the science and theory behind best practice for optimal patient care transitions; 2. articulate the facilitation of patient care transitions around dialysis, including patient education, role of palliative care, and conservative management; and 3. describe how we prepare patients for kidney transplantation and for life after a failed allograft.
