Course Enrollment is Limited!
Previous courses have been hugely successful and space is limited! Those preparing for recertification are increasing our numbers each year. Please register early to ensure your place in the 2007 course. The pre-registration deadline is Friday, August 10, 2007. Onsite registration may not be possible, due to space limitations.

Mail or Fax Registration
You may also submit your registration and payment to the ASN online, by fax, or by mail. The ASN does not accept telephone registrations. If you mail your registration, please allow two weeks for processing.

Fees may be paid to the ASN by check, Visa, MasterCard, or American Express. The ASN does not accept wire transfers. Please make your check payable to the American Society of Nephrology. All checks must be in U.S. dollars and drawn on a U.S. bank.
Registrations will be confirmed to all participants via email. Registration forms received without payment or complete credit card information cannot be processed.

Discounts for Previous Attendees!
The ASN is pleased to extend a discount to previous Board Review Course & Update participants from the past three years (2004, 2005, or 2006). If you are interested in a refresher course, these special rates are as follows:

* For all Fellows, we require the name of your Training Program Director and their daytime telephone number, to document your active fellowship status.
NOTE: To register as a member, please provide us with your ASN membership number, which is on the mailing label of the BRC brochure, the mailing label of JASN, or may be obtained from the ASN via email (email@asn-online.org). To secure a discounted registration fee, your ASN membership should be in place by Friday, July 27, 2007.
Non-Members: To become a Member of the ASN, please download the ASN Membership Application. Send your completed membership application and fees to us by Friday, July 27, 2007, in order to secure a discounted registration fee. If you register as a non-member and then apply for membership with the ASN, you will not be eligible for a refund of the difference in rates.
Workshop and Official Symposia Registration (at no additional cost)
Pre-registration is requested for all workshops and official symposia. Please register for breakfast, midday, and concurrent advanced workshops and the official symposia sessions. Space is limited and will be assigned on a first-come, first-served basis. Your course confirmation email will indicate the workshops and symposia for which you are registered. Onsite admittance to these sessions will be based on the availability of space. You may download a Workshop and Official Symposia Registration form from the ASN website, but you should be certain to submit both your course registration form and your workshop registration form at the same time to the ASN. Both course registration and workshop/official symposia forms are included together in the PDF at the top of this page.
Daily Luncheon Meals
For your convenience, a luncheon meal is available at a discounted cost of $30.00 each. You will need to purchase the luncheon meal tickets IN ADVANCE, so please indicate on the registration form which luncheons you want, and add their total cost to your registration payment. A ticket will be issued for each meal purchased. There can be no refunds for meals cancelled after Friday, August 10, 2007. Luncheon meals cannot be added onsite. A vegetarian option will be available each day to those who indicate this as their choice in the space provided on the registration form.
Registration Materials
Your registration materials, including your meeting badge, syllabus, and luncheon meal tickets (if applicable) will be available to you onsite at the ASN Registration Desk. Registration begins at 9:00 am on Saturday, August 25, 2007, in the Regency Foyer. Please have your confirmation email available as proof of registration and to receive your meeting materials.

Refunds for Cancellation
Cancellation notification and requests for refunds must be submitted in writing to the ASN by Friday, August 10, 2007. Refunds, less a $100 administrative fee, will be made after the conclusion of the course. Course expenses preclude our granting refund requests received after Friday, August 10.
