FASN Application Process
There are two pathways to FASN status, the clinical pathway and the investigator pathway. Although the standards for FASN status are high, the application process itself is simple. Each pathway has just five steps, which are detailed in the FASN brochure. ASN members applying for FASN status select and must meet the criteria for one of the pathways.
To apply for FASN status, complete and mail the application form, along with your CV. You may also fax the application to the ASN and e-mail or mail your CV separately. The ASN staff will acknowledge your application by e-mail and let you know if any additional information is required.
Once your application is accepted, you will receive a certificate attesting to your FASN status, along with a FASN pin. Your name will be published quarterly on the ASN website and in the newsletter ASN Highlights, distributed each year during Renal Week.
FASN status is valid for five years, during which time you can use the letters "FASN" in connection with your professional activities. During the five-year term, you are expected to maintain the published criteria for FASN status. You will receive a renewal application before the end of your five-year term.
