Predicting Drug Disposition in CKD: Not Just a Renal Affair

Session Description

Renal elimination of many medications decreases as GFR declines; however, it is difficult to predict drug disposition in advanced kidney diseases. Other factors that influence drug disposition include the effects of kidney diseases on nonrenal clearance and drug transporters. Physiologically based pharmacokinetic (PBPK) modeling integrates physiologic and drug-dependent information derived from in vitro, in silico, and in vivo data to predict pharmacokinetic profiles. This session explores factors affecting drug disposition and application of PBPK models.

Session Objectives

Upon completion of this session, the participant will be able to: 1. review data on select drug metabolizing enzymes and drug transporters responsible for renal and nonrenal clearance of medications; 2. describe the mechanisms of altered drug metabolism in conditions of uremia and RRT; 3. discuss the effects of kidney diseases on renal and Nonrenal drug transporters and implications in CKD; and 4. explain PBPK modeling and specific applications of modeling to predict drug disposition in CKD.
