Tubulointerstitial Disease: Double Trouble in the Kidney

Session Description

Tubulointerstitial lesions remain an important area of nephrology and should be recognized by clinicians. Mesoamerican nephropathy is a new and poorly understood tubulointerstitial disease. IgG4 is an autoimmune disease with multiorgan involvement, with tubulointerstitial and glomerular involvement. HIV-associated tubulointerstitial disease is comprised of an interesting group of immune-related lesions. Finally, crystalline-induced tubulointerstitial disease is an important cause of kidney diseases, which may actually be the result of an inflammatory response rather that tubular obstruction. This session addresses these topics.

Session Objectives

Upon completion of this session, the participant will be able to: 1. describe the epidemiology, clinical features, and potential causes of Mesoamerican nephropathy; 2. recognize the clinical presentation, radiologic and pathologic causes, and treatment of IgG4-related tubulointerstitial disease; 3. recognize HIV-associated tubulointerstitial lesions—diffuse infiltrative lymphocytosis syndrome (DILS), immune reconstitution inflammatory syndrome (IRIS), and plasmacytic infiltration; and 4. discuss the mechanism of kidney injury associated with crystalline-related nephropathy.
