Hypoxia-Inducible Factors: The Future of Anemia Management in CKD?

Session Description

Anemia is a commonly treated complication of CKD. Erythropoiesis-stimulating agents and intravenous iron have been used in combination to manage anemia for many years. However, recent clinical trials and epidemiologic studies have demonstrated that balancing the benefits and risks of these agents may be difficult.

Transformative research over the last decade has elucidated the central role of hypoxia in the regulation of anemia. Hypoxia-inducible factors (HIFs), stimulated by the oxygen-sensing pathway, regulate both erythropoiesis and iron utilization. Currently, agents that stabilize HIFs are in advanced stages of clinical testing. These agents may soon be available to provide a comprehensive approach for the treatment of anemia. It is important for the nephrology community to understand differences among the various HIF stabilizers with respect to dosing frequency, efficacy at raising hemoglobin, iron utilization, and safety.

Session Objectives

Upon completion of this symposium, the participant will be able to: 1. discuss the benefits and risks of current approaches to anemia management in CKD; 2. describe the role of the hypoxia pathway in the regulation of erythropoiesis, iron metabolism, and inflammation; and 3. discuss the efficacy and safety of the HIF stabilizers currently in phase II and III clinical trials.
