Session Description

CKD is an underrecognized and growing public health issue. Earlier identification of patients with CKD and the subsequent potential opportunities to improve clinical care is needed to slow CKD progression and improve clinical outcomes. This session presents a comprehensive framework for a population health-based approach to kidney diseases. Particular emphasis is placed on utilizing electronic health records (EHR) to identify CKD, measuring quality in kidney care, and team-based medication management. Value-based care management programs are also discussed.
