Current Therapies and Treatment Responses in Primary Focal Segmental Glomerulosclerosis and IgA Nephropathy

Session Description

Primary FSGS and IgAN remain important causes of progressive kidney disease and ESKD. One urgent unmet need is personalized, effective, and safe therapies that delay or prevent kidney failure. Evaluations of multiple therapeutic agents for primary FSGS and IgAN in recent clinical trials have yielded mixed results and inconsistent clinical benefit. The advances in treatment for these two glomerular diseases have been hindered by a number of factors: clinical and histological heterogeneity within each disease entity, incomplete understanding of complex molecular mechanisms, varying duration of therapy in different studies, inconsistent definitions of treatment response, and different outcomes evaluated by different clinical trials.

Despite these limitations, continued progress has been made in the understanding of disease mechanisms and the discovery of new therapeutic targets in FSGS and IgAN. The efficacy and safety of several novel therapies are currently being evaluated. In addition, ongoing efforts explore surrogate outcomes, such as proteinuria and eGFR slope, and their association with long-term kidney survival.

This symposium reviews the current therapeutic options for FSGS and IgAN and discusses various measures of disease response and treatment goals.
