Session Description

Person-centered care is key to enabling people on dialysis to achieve their goals. This session explores the information to share with patients when making important decisions related to dialysis care, such as when to start and end dialysis therapy.

• Identify factors to consider when starting dialysis
• Explain how shared decision-making enables changes in peritoneal dialysis prescription and transition to home hemodialysis
• Outline the decision-making needed for young people transitioning from pediatric to adult care
• Describe processes related to decision-making about discontinuing dialysis at the end of life
