Recently, every Medicare Administrative Contractor (MAC) nationwide has issued an identical draft Local Coverage Determination (LCD) that would limit the provision of dialysis more than three times per week exclusively to patients with a few specific acute conditions. The draft LCD proposes that a claim linked to a Plan of Care (POC) that includes a dose of dialysis of more than three treatments per week for any chronic condition, or for an acute condition not included on the list, will be denied. Below is a sample letter for you to use to contact each MAC and a list of instructions for emailing your letter and the deadline for submission. In order for your letter to be seen by each MAC, you must email it to each one.
