Home Hemodialysis Therapies in Older and Frail Adults

Session Description
Many older adults receiving hemodialysis are not able to care for themselves in their own home, and often have limited, if any, family support, which leads to placement in a long-term care facility. Due to the medical and physical instability that often exists at the time of hospital discharge, many patients receiving hemodialysis are admitted to a skilled nursing facility. With people living much longer and an increasing number of patients with ESKD, having facilities that provide a "home" for these patients that include onsite hemodialysis has been shown to have many benefits.

- Discuss the management of older patients who are dependent on hemodialysis
- Outline the logistical support needed with an on-site hemodialysis unit and nursing home employees
