Background

Height-adjusted TKV via Magnetic Resonance Imaging (MRI) is standard for ADPKD but costly.We assessed a novel ultrasound(US)-based modified Simpson method,adapted from echocardiography,to improve volume accuracy over ellipsoid estimates.

Methods

This cross-sectional study included 85 ADPKD patients from Al-Saegh Center, Karbala,Iraq.Kidney length (Z),width (X),and depth (Y) were measured at six transverse levels via US.Volume per disc was calculated as TKV = ∑ π × (Xi/2) × (Yi/2) × 1 cm.Bilateral volumes were summed and adjusted for height to yield htTKV.Patients were stratified by Mayo risk groups.ROC,Pearson correlation, and ANOVA were used to compare this method with MRI and ellipsoid-based US.

Results

Among 85 patients,htTKV stratified three risk groups.An inverse correlation was seen between htTKV and eGFR (r = –0.45, p < 0.01).eGFR differed by group (ANOVA p = 0.04).ROC (Figure 2) showed AUC 1.00 for Mayo 1D–1E.The disc method showed 94% sensitivity, 99% specificity,outperforming ellipsoid US.

Conclusion

This US method estimates htTKV, correlates with MRI,and stratifies ADPKD by Mayo class.Multicenter validation is underway.We thank the Alsaegh Research Group and patients.
