Integrating Lifestyle and Nutrition into CKD Care: Physician-Led Kidney Nutrition Program at Yale

Introduction

Chronic Kidney Disease (CKD) is a prevalent condition that necessitates a multifaceted approach, including medical management, dietary modifications and patient-education. The integration of nutrition programs into kidney clinics has shown promise but remains underutilized. This case report explores the clinical course of a CKD patient participating in a new physician-led kidney nutrition program, with a focus on implementation of science principles to optimize care and outcomes.

Case Description

The patient is 58 year old male with stage 3a chronic kidney disease (CKD) due to autosomal dominant polycystic kidney disease (ADPKD) who came for initial evaluation in the Lifestyle and Nutrition for Kidney Clinic. Prior to arriving in clinic, the patient had filled out lifestyle and nutrition assessments to help determine his current practices and areas for improvement. The two pillars that were identified to improve upon were nutrition and sleep during the patient visit. Personalized nutrition strategies based on the KDOQI guidelines were integrated into his current diet. We helped change patient nutrition focus from a keto diet to a plant forward whole food diet. We helped personalize his dietary protein, sodium and water intake to help with his PKD. Regarding his sleep, goals were established with the patient as to his own preferences and shared decision-making led to the establishment of sleep goals and a referral to a sleep specialist.

Discussion

This case highlights how a physician-led nutrition clinic can elevate the role of lifestyle interventions in CKD care. By embedding evidence-based nutrition guidance into routine nephrology care, patients may become more engaged in modifying risk factors that affect disease progression. Furthermore, integrating other specialists--such as dieticians, sleep experts, and social workers--creates a comprehensive and patient-centered model that addresses both clinical and behavioral deteriminants of health. While such multidisciplinary teams are commonly employed in transplant programs or kidney stone clinic, they are underutilized in general nephrology, presenting an opportunity for broader implementation.
