Ultrasound-Guided vs. Blind Percutaneous Placement of Peritoneal Dialysis Catheters: Single-Center Cohort Study of One-Year Catheter Survival

Background
Peritoneal dialysis (PD) catheter malfunction is a common cause of technique failure. Image guidance is advised during percutaneous insertion to improve safety, but its effect on long-term catheter survival remains uncertain. We compared one-year catheter patency after ultrasound-guided (US-guided) vs blind PD catheter placement.
Methods
Adults who underwent percutaneous PD catheter insertion at Ramathibodi Hospital between 1/1/10 and 3/31/22 were retrospectively reviewed. Outcomes were catheter survival at 12 months, complication-free survival, infection rates, and catheter removal. Multivariable Cox regression identified predictors of catheter failure.
Results
68 patients were included (US-guided 32; blind 36). Groups were comparable except for higher prevalence of prior stroke (31.2 vs 8.3%) and anticoagulation (25 vs 6%) in the US-guided cohort (p=0.04, 0.04). US guidance achieved intrapelvic tip positions (66 vs 39%, p=0.03). Twelve-month catheter patency was 90% with US guidance and 65% with blind placement (p=0.02). US guidance independently predicted catheter survival (HR 5.88, p=0.04). Complication-free survival, infection rates, and catheter removal did not differ between groups.
Conclusion
US-guided PD catheter placement yields significantly better one-year patency than blind placement, without increasing infectious or mechanical complications. US guidance is a practical, safe, and effective option for percutaneous PD catheter insertion.
