Background
Severe tricuspid regurgitation (TR) can lead to significant organ dysfunction, including renal impairment. However, data on the impact of TR on kidney function and outcomes after interventional treatment remain scarce. This study aims to investigate the initial renal function of patients with high-grade tricuspid valve insufficiency undergoing interventional Transcatheter edge-to-edge repair (T-TEER) procedure and the course of kidney function after intervention.
Methods
This retrospective, observational single-center study included n=181 consecutive patients with severe, massive and torrential TR who underwent T-TEER implantation conducted at Robert Bosch Hospital Stuttgart (Germany) between March 2021 and February 2023, with median follow-up of 125 days (IQR 6-506) up to 1280 days. Follow up included all routine records in our EHRs. Cases with insufficient data were excluded.
Results
Among the 181 patients analyzed, baseline renal function showed a median estimated glomerular filtration rate (eGFR, CKD-EPI) of 53 mL/min/1.73m2 [41-65]. Chronic kidney disease (CKD) was present in more than two thirds (64%) of patients, with 30% classified as CKD stage G3a, 26% as G3b, 8% as G4, and 5% were on dialysis (n=9).
Renal function, as measured by eGFR, improved from 52.7 ± 17.3ml/min/1.73m2 to 59.5 ± 18.8ml/min/1.73m2 (p < 0.0001), and eGFR-slopes were significantly less steep (p < 0.0001) (Figure 1). Cumulative hazard of AKI events were less frequent in lower CKD-Stages.
Conclusion
Our data indicate that the CKD prevalence in patients with high-grade TR is remarkably high, and the findings suggest that renal function improve over time in patients following intervention. These observations highlight the need for further prospective studies to systematically evaluate renal function outcomes after T-TEER.
