Background
Peritoneal dialysis (PD) offers a home-based alternative to hemodialysis, currently utilized by approximately 8% of dialysis patients in Israel. PD allows for daily treatment, potentially preserving residual renal function and offering greater lifestyle flexibility. This pilot study evaluates a virtual clinic model to enhance PD care and patient outcomes.
Methods
This pilot program integrates remote monitoring tools and proactive communication to improve patient care. Fifteen PD patients were enrolled and utilized: 1) Daily remote monitoring of blood pressure and weight via the DATOS system; 2) Weekly questionnaires assessing for potential complications; 3) Personalized educational messages and reminder; and 4) monthly quality-of-life assessments.
Results
Fifteen patients completed the pilot program. On average, caregivers initiated 10 chat messages per patient per month, while patients sent an average of 7 messages that were addressed by the care team. The average medical team response time was 4 days. Patient satisfaction was high. Most interventions involved blood pressure management, addressing technical difficulties with PD, managing weight fluctuations, and pain control. The care team utilized platform data during monthly in-person visits. Educational resources provided to patients included videos on PD technique, dietary guidelines, and refresher training materials.
Conclusion
This virtual clinic pilot demonstrates the feasibility and operational effectiveness of a remote monitoring and communication system for PD patients. The model aims to enhance PD care by facilitating early detection of complications, improving patient safety and satisfaction, reducing hospital visits, and providing continuous support and medical advice. Ongoing data collection and patient feedback will further optimize the system and inform its broader implementation.
