Effectiveness of Home Hemodialysis Therapy with NxStage Machine on Albumin and Mineral Metabolism Parameters: Experience in the Gulf Corporation Council (GCC)

Background
Home hemodialysis is most prevalent renal replacement therapy as ESRD remains a challenge, affecting patient’s quality of life. HHD enables patients to access frequent dialysis, for longer durations, showing better control over metabolic parameters- nutritional status and mineral metabolism. Nurse Assisted Home Hemodialysis (NAHHD) is an innovated modality in the GCC, treats multi-comorbid, homebound and debilitated patients. We present HD adequacy and effectiveness of treatment on albumin levels and metabolic status.
Methods
Study includes 506 patients at NAHHD treated with NxStage HD machine at home and long-term care facility, for >3 months. HD adequacy with a target standardised Kt/V ≥2.1. Alb, Ca, PO4, PTH were measured as key performance markers with other HD parameters.
Results
Female- 53.8%, Male- 47.2%. Avg. age: 68.72 ±15.49 yrs. (Age ≥60: 79%). Avg. comorbidity: 4.43 ± 1.11 (min-3, max-8). Vascular access: AVG/ AVF- 34%, PC- 66%. Above 93% of patients achieved Ca level in target range, PO4 level- 95% in range, managed PTH. Alb level illustrated in graph 1. Yearly avg. of the biological KPI’s in Table 1. Result shows managed KPI levels.
Conclusion
This study showed effective outcomes of NAHHD therapy with significant values of the target biochemical markers and HD adequacy in the patients. Using portable HD machine is safe, efficient, with better clinical and biological outcome.
