Background
Expanded hemodialysis (HDx) is a substantial advancement in kidney replacement therapy, utilizing medium cut-off (MCO) membrane to improve the clearance of middle- and large-molecular-weight uremic toxins. The lower resource requirements of HDx may position it as a potentially resource-effective alternative.
Methods
We conducted a micro-costing analysis from the healthcare provider perspective, focusing on adult patients with end-stage kidney failure on maintenance dialysis. The analysis used individual treatment sessions to compare HDx and OL-HDF. Direct medical costs were calculated in euros using administrative data from a dialysis provider in Spain. Costs of personnel, medications, dialysis equipment, and monitors were excluded due to similar usage across modalities. A Monte Carlo simulation estimated water consumption differences. HDx prescriptions used a Theranova dialyzer (MCO) with a QD of 500 mL/min (120 L/session) and for OLHDF Polyflux dialyzer with a total QD flow of 625 mL/min (520 mL/min and 105 mL/min, substitution volume), leading to a total water consumption of 150 L per session.
Results
Assuming a €2 higher cost of Theranova compared to high-flux dialyzer, HDx demonstrated a net savings of €2.70 per session relative to OL-HDF, primarily due to the elimination of the OL-HDF line and reduced water usage. The Monte Carlo simulation estimated that a cohort of 1,000 patients undergoing 156 HDx sessions annually could use 23.4 million liters of water less over five years.Figure 2.
Conclusion
HDx with Theranova offers significantly lower resource use compared to OL-HDF by reducing the need for specific consumables and water consumption. These findings support the potential benefit of HDx as an efficient dialysis modality which responsibly uses the planet's resources.
