Background

This study included 2,043 type 2 diabetes patients(2022-2023). CI-AKI was defined by serum creatinine criteria. Multivariable analyses identified risk factors, with ROC/AUC evaluating predictive performance. RCS and a nomogram quantified nonlinear relationships and individualized risk.

Methods

This retrospective cohort included 2,043 type 2 diabetes patients from two tertiary centers (2021-2023). CI-AKI was defined by serum creatinine criteria. Multivariable analyses identified risk factors, with ROC/AUC evaluating predictive performance. Restricted cubic splines (RCS) and a nomogram quantified nonlinear relationships and individualized risk.

Results

CI-AKI occurred in 273 patients (13.4%). Independent predictors included GNRI (OR=0.912) and PNI (OR=0.812). PNI had superior discrimination (AUC=0.871, P<0.001) vs. GNRI (AUC=0.755, P<0.001)) and CONUT (AUC=0.547). The nomogram achieved 80% accuracy.

Conclusion

GNRI and PNI predict CI-AKI and adverse outcomes in diabetic patients undergoing CAG/PCI better than CONUT.
