Background

Frailty and other geriatric impairments are associated with adverse outcomes in older people living with chronic kidney disease (CKD). Comprehensive Geriatric Assessment (CGA) is the standard of care of older people. The aim of this study was to establish research outcomes that can demonstrate effectiveness of CGA in the advanced kidney care setting.

Methods

Focus groups were performed with older people (>65 years) with advanced CKD (n=3), caregivers (n=3) & healthcare professionals (n=2). A patient advisory group informed the content and structure. Four interviews were performed with non-English speaking participants. Focus groups and interviews were recorded and transcribed. Two researchers independently coded data. An initial framework was developed based on the aims & objectives of the focus groups. The analysis led to the identification of several key themes by mapping & interpreting the coded groups. A final focus group with invited participants from the other groups was conducted. Themes identified were ranked in order of importance using nominal group technique.

Results

In total, 50 participants were involved in the focus groups: 16 patients (median age 80 years [IQR 76-84]), 17 caregivers, &17 healthcare professionals. The final focus group themes in ranked order were: 1. Enabling meaningful life participation; 2. Patient/carer-centred planning & choices; 3. Timely, tailored information and education; 4. Flow of care & joined-up working; 5. Future care & choices; 6. Carer experience & support. Enabling meaningful life participation, the most important priority, included the following subthemes: maintaining enjoyed activities & independence, meaningful life participation, & effective symptom management to maximise abilities & life engagement.

Conclusion

This multicentre stakeholder research study has identified a core set of outcomes important to older people living with advanced CKD & frailty, their caregivers, & healthcare professionals involved in their care. Findings will inform the development of outcome measures for a UK national study evaluating the effectiveness of CGA adapted to the advanced kidney care setting.
