Background
SGLT-2 inhibitors (SGLT-2i) have demonstrated cardio- and renal-protective benefits and are increasingly adopted to halt CKD progression. However, global awareness and distribution remain disproportionate. Using Google Trends and GBD 2023 prevalence data, we highlight disparities in SGLT-2i awareness relative to CKD burden by nation and identify regional drug preferences.
Methods
Objectives: (1) Identify global disparities in SGLT-2i awareness; (2) Compare awareness to CKD prevalence by region; (3) Identify the most popular drug per region.
Google Trends data over 5 years captured SGLT-2i search interest using generic and brand name terms. Age-standardized CKD incidence and prevalence were obtained from GBD 2023. "Awareness gaps" were flagged where CKD burden exceeded the 75th percentile but search volume was undetectable.
Results
Of 204 countries with CKD data, only 54 had any SGLT-2i awareness signal. Among high-burden countries (prevalence ≥11,475 or incidence ≥333 per 100,000), 39 showed no awareness. The ten most affected nations—including Haiti, Panama, Guatemala, and Nicaragua—had prevalence above 12,000 and incidence exceeding 395 per 100,000. Among countries with signals, Jardiance was most searched (65%), followed by Farxiga (24%), dapagliflozin (22%), and empagliflozin (16%). Jardiance dominated North America, Western Europe, and Southeast Asia; dapagliflozin led in Russia and India; empagliflozin was most searched in the Middle East.
Conclusion
A critical gap exists between CKD burden and SGLT-2i awareness, particularly across South America and Africa. The dominance of the search term Jardiance compared to generic SGLT-2 inhibitor terms suggests that people are more familiar with brand names than with lower-cost generic options. Targeted efforts are needed to improve global accessibility and distribution of these therapies.
