Background

Neonates born with end-stage kidney disease (ESKD) require specialized considerations for nutrition, fluid balance, electrolyte management, and pharmacological therapies. This novel curriculum aims to provide ESKD education to neonatology fellows and advanced practice providers. Curriculum development followed Kern's six steps and was implemented in an in-person, half-day pilot session with 13 course leaders and 14 learners. The primary objective is to evaluate if the learners' understanding and confidence around neonatal ESKD care improved after participation in the pilot.

Methods

The study utilized a retrospective paired pre- and post-test assessment for learners to rank their understanding and confidence. This ensured a paired dataset and mitigated the effects of response-shift bias. All learners were invited to participate in the study. Data were analyzed with descriptive statistics given the small sample size.

Results

7 of 14 (50%) learners completed the assessment. Analysis included 7 items around comprehensive neonatal ESKD care (Table 1). Medians were calculated for the pre- and post-course item responses (Figure 1). The participants had self-reported improvements in all categories (100%) following course participation. The largest perceived learner benefit was surrounding counseling families around the early childhood course of ESKD.

Conclusion

Neonatal providers perceived benefits in both understanding and confidence around comprehensive neonatal ESKD care after attending the pilot, which provides a foundation for future course growth.

Level of understanding for the following topics:
(1=Novice, 2=Beginner, 3=Intermediate, 4=Advanced, 5=Expert)
Item A: Causes of neonatal kidney disease
Item B: Nutritional considerations for neonates with ESKD on dialysis
Item C: Process around transitioning a neonate
Confidence in the following areas:
(1=Very Unconfident, 2=Fairly Unconfident, 3=Neutral, 4=Fairly Confident, 5=Very Confident)
Item D: Fluid balance management for neonates with ESKD on dialysis
Item E: Caring for a neonate maintained on peritoneal dialysis
Item F: Caring for a neonate maintained on CRRT
Item G: Counseling families around the early childhood course of ESKD
