Background
Epidemiology data for focal segmental glomerulosclerosis (FSGS) are limited and substantially impacted by individual healthcare settings and/or accessibility of kidney biopsy in different global regions. Furthermore, many analyses report the frequency of FSGS among kidney biopsy cohorts rather than true population-based data. This systematic review aimed to identify recently published studies that reported the epidemiology of FSGS in both the United States and globally.
Methods
The review included searches of the MEDLINE and Embase databases for the past 5 years and several congresses from the prior 2 years. Searches used terms related to FSGS and epidemiology (incidence, prevalence, and mortality). Eligibility criteria included observational studies published in English that examined the epidemiology of FSGS in any country or region worldwide. Studies that assessed the frequency of FSGS from kidney biopsy data and did not convert findings to a population-based estimate were excluded.
Results
A total of 1205 unique articles were screened by 2 independent researchers by title and abstract; 127 full-text articles were subsequently evaluated and 21 met the inclusion criteria. Most included studies assessed the epidemiology of FSGS within the US, Canada, Japan and Europe. Prevalence was reported in US-based studies only and showed a consistent increase in FSGS over time reaching approximately 200 cases per 1 million population in recent years (across all disease subtypes: primary, secondary, and genetic). Incidence rates were less consistent with respect to time-based trends and the most recent estimates across multiple countries ranged from approximately 0.4 to 2.1 per 100,000 population in adults (all disease subtypes). In addition, considerably higher incidence and prevalence rates were observed among Black populations and older age groups. Five- and 10-year mortality rates were approximately 10-15% and 20-25%, respectively, although mortality and survival data were heterogeneous and varied widely across studies.
Conclusion
The rising prevalence of FSGS, particularly among high-risk demographic groups, underscores the need for improved epidemiological monitoring and effective treatment options to mitigate the increasing burden of FSGS.
