Introduction

Reninoma is a rare tumor of the juxtaglomerular cells that leads to autonomous secretion of renin that leads to severe hypertension, hypokalemia, metabolic alkalosis. Surgical removal is often curative.

Case Description

This is a 36-year-old male presented to the ER with GI bleed at age 33 and was incidentally diagnosed with hypertension with blood pressure of 189/108 mmHg with hypokalemia, metabolic alkalosis and was discharged on nifedipine, labetalol, eplerenone and potassium tabs. Plasma renin activity was elevated at 66 ng/ml/hour, aldosterone was high-normal at 28 ng/dl.Rest of the work up was normal. Due to trasnaminitis Gastroenterology ordered MRI of abdomen which showed 1.7 cm solid right upper pole renal lesion(Image 1). He underwent laparoscopic resection of this tumor (Image 2). Pathological examination of this resected specimen was consistent with diagnosis extraglomerular renal cell tumor. Within 4 to 6 hours after resection of this tumor his blood pressure was normal and no longer required anti-hypertensives.

Discussion

Reninoma should be considered and CT or MRI imaging should be done in Hypertension with elevated renin and aldosterone. Laboratory findings, pathology of the resected tumor and complete resolution of Hypertension after resection of the tumor confirms the diagnosis of Reninoma. Endocrine Hypertension presents a rare oppurtunity to cure Hypertension with successful resection of the tumor which avoids life long use of anti-Hypertensive medicaitons.
