Prevalence and Temporal Trends of Cardiovascular-Kidney-Metabolic Syndrome Among Chinese Inpatients Undergoing Kidney Biopsy

Background

Cardiovascular-kidney-metabolic(CKM) syndrome involves interconnected metabolic,chronic kidney and cardiovascular disease. While renal biopsy is the gold standard for renal histologic diagnosis,CKM syndrome epidemiologic data in biopsy populations are limited.This study investigated CKM syndrome prevalence and temporal trends in Chinese renal biopsy inpatients.

Methods

We retrospectively analyzed 1902 adults who underwent their first renal biopsy between 2011 and 2019. Clinical, laboratory, and renal histopathological data were collected.Patients were stratified into two time periods:2011–2015 and 2016–2019.CKM stages 0–2 were defined as the low-risk group,and stages 3–4 as the high-risk group.Statistical analyses were performed using SPSS,with p < 0.05 considered statistically significant.

Results

Patients had a mean age of 43.82±12.81 years and a male-to-female ratio of 1.19:1.CKM staging differed significantly between the two time periods.Compared with the 2011–2015 period,patients in the 2016–2019 period had a higher proportion of high-risk group,membranous nephropathy(MN),and diabetic kidney disease(DKD),exhibited a higher prevalence of hypertension(HT),diabetes mellitus(DM),hyperlipidemia(HLP),cerebrovascular disease(CVD), and arterial diseases(p<0.05).They were also older, had higher systolic blood pressure(SBP),lower estimated glomerular filtration rate(eGFR),higher serum uric acid(UA),and higher urine protein-to-creatinine ratio(PCR)(p<0.05)(Table1).

Conclusion

Over time, the patients undergoing renal biopsy were older,presented with more severe renal dysfunction and advanced CKM stage,and had a higher prevalence of concomitant metabolic and vascular complications.These findings highlight the importance of enhanced monitoring and targeted management of metabolic disorders and vascular diseases among hospitalized patients with kidney disease.
