Background

Osteoporosis and CKD MBD are considered as overlapping entities increasing mortality, fracture risk. The aim of the study is to analyse the prevalence and independent risk factors of osteoporosis and bone turnover in CKD on MHD patients

Methods

It is a Multicentric cross-sectional observational study done on 855 MHD in 17 centres.Details collected were :Clinical, biochemical, and bone density data - iPTH, Ca, Phos, Serum albumin, and a USG-based bone density study of the foot. BMD - < –2.5 indicates osteoporosis.Fracture history, comorbidities, dialysis vintage, and medication use noted.Spearman's correlation and multivariate linear regression were used to identify predictors of T-score.

Results

A total of 855 CKD on MHD were included .Patients were mostly male 75% with mean age around 49+/-8 years. 59.7% had high turnover bone disease. The prevalence of osteoporosis was 30.7%.Fractures present in 11.1% of patients and were not significantly associated with bone turnover pattern or osteoporosis status (p = 0.318).Calcium and Vit D supplementation was not significantly associated with osteoporosis .Osteoporosis was common in female patients (p = 0.001).On multivariate regression, age and serum albumin (β = 0.080, p = 0.032) were independent predictors of T-score, while iPTH did not reach statistical significance (p = 0.404).

Conclusion

This large multicentric study from India has uncovered the bone disease prevalence data on osteoporosis and mineral bone disorders in a low socioeconomic status population.
Women are more prone to fractures compared to men.
Early screening strategies, correction of Ca P , fracture prevention programs, to detect early bone disease should be implemented
Injection Denosumab 60 mg s.c will be given to the patients with osteoporosis and will be followed up
