Kidney Biopsy During Pregnancy or the Postpartum Period

Background

This study evaluated whether performing kidney biopsy during pregnancy versus waiting until postpartum period influences long term kidney survival and maternal fetal outcomes.

Methods

22 year retrospective cohort study (January 2002 to November 2024), including women aged above 18 who underwent native kidney biopsy during pregnancy or within three months postpartum. Outcomes included kidney survival (time to dialysis), maternal and fetal outcomes, histopathology patterns, and biopsy related complications.

Results

13 women underwent biopsy during pregnancy and 16 postpartum across all trimesters. Lupus nephritis was the most common histopathologic diagnosis in both cohorts. Proteinuria was the predominant indication for biopsy. Biopsy findings during pregnancy led to management changes in ALL cases, including initiation of high dose corticosteroids, iatrogenic preterm delivery, or pregnancy termination when necessary. In contrast, only 50% of postpartum biopsies altered management. Long term kidney survival did not differ significantly.

Conclusion

Kidney biopsy during pregnancy demonstrated acceptable safety and yielded high diagnostic utility, directly influencing clinical management in all antenatal cases.
