Medicare Prescription Drug Coverage of Medications for IgAN

Background
IgAN treatment has changed significantly in recent years, with new medications, clinical trial evidence, and updated guidelines. For patients at risk for progressive loss of kidney function, KDIGO guidelines stress simultaneous treatment to 1. target IgAN-specific factors (first-line Tarpeyo) and 2. slow progression of chronic kidney disease. This project evaluated Medicare prescription drug plans for coverage of IgAN medications.
Methods
All 2026 Medicare stand-alone prescription drug plans (PDP) and Medicare Advantage Plans with drug coverage (MA-PD) available in NY state were identified via the CY2026 Landscape file on cms.gov. Coverage of medications was determined using medicare.gov/plan-compare. Medications evaluated included: SGLT2 inhibitors (dapagliflozin, Invokana (canagliflozin), Jardiance (empagliflozin), and Steglatro (ertugliflozin)), Fabhalta (iptacopan), Tarpeyo (budesonide), Vanrafia (atrasentan), Voyxact (sibeprenlimab-szsi), and Filspari (sparsentan).
Results
n=120 MA-PD and n=10 PDP plans were available in NY state in 2026. All PDP and MA-PD plans covered at least one SGLT2 inhibitor. No PDP covered Fabhalta, Filspari, Tarpeyo, Vanrafia, or Voyxact. Among the 62 counties in NY, there were 14 (22.6%) counties with no available plans that covered Tarpeyo. The percentage of MA-PD plans covering each medication and the number of counties with coverage available for each of the medications is shown in the Table.
Conclusion
Limited coverage of IgAN medications may lead to delayed or suboptimal treatment among patients with Medicare prescription drug plans.In particular, the lack of coverage for the first-line treatment (Tarpeyo) is concerning. Interventions to improve medication access are warranted.
