Background
Aim: To investigate the effect of sodium-glucose cotransporter-2 inhibitors (SGLT2i) on renal parameters in patients with IgA nephropathy (IgAN).
Methods
Included patients older than 16 years with histopathological confirmation of IgAN and were studied retrospectively. Demographics, comorbidities, clinical and laboratory findings at the time of diagnostic kidney biopsy and throughout follow-up, treatment regimens, and therapeutic responses were recorded. Clinical phenotypes of IgAN were categorized as follows: A) isolated microscopic hematuria, B) hematuria and proteinuria, and B) hematuria, proteinuria, and impaired renal function.
Results
Up to the present, 94 patients have been included in the study, with a median age of 48 years (IQR: 23). Fifty-six patients (60%) were males, 45 (48%) had arterial hypertension, and 10 (11%) had type 2 diabetes mellitus. Twenty two patients (32%) presented with isolated microscopic hematuria, 40 (57%) with hematuria and proteinuria, and 8 (11%) with hematuria, proteinuria, and impaired renal function. Seventy-seven patients (82%) were receiving renin–angiotensin–aldosterone system inhibitors (RAASi), while 52 (55%) had received immunosuppressive therapy. Overall, 31 patients (32.9%) were treated with SGLT2 inhibitors.
Patients receiving SGLT2i were more frequently males (p=0.001), were receiving RAASi at the time of SGLT2i initiation (p=0.004), and had significantly higher baseline 24-hour proteinuria at diagnosis compared with those who did not receive SGLT2i [2270 mg (IQR: 3500) vs. 910 mg (IQR: 1195), p=0.003]. Baseline mean eGFR in the overall cohort was 81 (± 45) mL/min/1.73 sqm, declining to 69 (± 47.5) mL/min/1.73 sqm after a median follow-up of 61 months (IQR: 122). Accordingly, the relative reduction in 24-hour proteinuria was greater in
the SGLT2i group [Δproteinuria: −601 mg (2578)] compared with patients who did not receive SGLT2i [Δproteinuria: −390 mg (110)] during the same period.
Conclusion
Based on our preliminary results, treatment with SGLT2 inhibitors appears to confer an additional beneficial effect on proteinuria reduction in patients with IgAN.
