Background

Bariatric surgery improves obesity-related metabolic risk factors linked to chronic kidney disease (CKD). We evaluated stage-specific associations between prior bariatric surgery and CKD outcomes.

Methods

Retrospective cross-sectional study using 2022 National Inpatient Sample. Adult admissions categorized by prior bariatric surgery. Outcomes: any CKD, CKD stage 3, CKD stages 4–5, and ESRD. Multivariable logistic regression estimated adjusted odds ratios (aORs). Interaction and sensitivity analyses included obesity, diabetes, IPTW, and propensity matching.

Results

Among 6.58M admissions, 66,776 (1.0%) had prior bariatric surgery. Bariatric surgery was associated with lower odds of CKD stages 4–5 (aOR 0.74, 95% CI 0.63–0.86), but not CKD stage 3 or ESRD. Associations were strongest among patients with obesity (aOR 0.70) and diabetes (aOR 0.69). Findings were consistent across secondary analyses.

Conclusion

Bariatric surgery was associated with lower odds of advanced CKD, particularly among metabolically high-risk populations, supporting further study of metabolic surgery and CKD progression.
