Background

Pegmolesatide is a novel once monthly erythropoietin mimetic for renal anemia. This interim analysis evaluated hemoglobin (Hb) improvement following pegmolesatide initiation in dialysis patients.

Methods

This interim analysis included 75 dialysis patients with renal anemia who completed 24 weeks of follow-up in an ongoing multicenter study evaluating once-monthly pegmolesatide. Hb levels were assessed at baseline and at weeks 4, 12, and 24. Endpoints included Hb values, changes from baseline (ΔHb), and cumulative attainment rates for Hb >100 g/L, defined as achievement at least once up to each visit. Due to the limited sample size, no between-group comparisons were performed. Results are reported descriptively.

Results

The mean age was 53.0 years, and 54.7% were male. Regarding dialysis modalities, 52.0% received hemodialysis and 46.7% received peritoneal dialysis. Additionally, 92.0% had prior treatment history for renal anemia. Mean baseline Hb was 89.8 g/L. Mean Hb increased to 91.3 g/L at week 4 (ΔHb +1.9 g/L), 100.9 g/L at week 12 (ΔHb +11.0 g/L), and 104.1 g/L at week 24 (ΔHb +14.1 g/L). Progressive increases in cumulative attainment were observed over time. By week 12, 56.0% of patients had achieved Hb >100 g/L at least once, increasing to 74.7% by week 24.

Conclusion

In this descriptive 24-week interim analysis of 75 dialysis patients with renal anemia, pegmolesatide initiation was associated with progressive Hb increases and cumulative target attainment over time, including those with prior renal anemia treatment history. Although the sample size is modest, these findings illustrate the temporal pattern of Hb change following pegmolesatide initiation. (ChiCTR2400093814)
