Intracranial Aneurysm Characteristics in Patients with PKD and Other Kidney Transplant Recipients

Background
The need for intracranial aneurysm (ICA) screening in polycystic kidney disease (PKD) patients following kidney transplant (tx) requires further clarification.
Methods
The Wisconsin Allograft Recipient Database (WisARD), between 1/1/1994-12/31/2021, was queried for terms including “cerebral aneurysm” and “cerebral aneurysm rupture” to identify potential “PKD” and “Other” (non-PKD) kidney tx recipients (KTRs) who may have history of ICA and/or ICA rupture. A chart review was conducted to further delineate ICA related history, and risk factors for ICA.
Results
Our preliminary analysis confirmed 29 PKD and 15 Other KTR’s with ICAs. Notably, in PKD KTRs, ICAs were most likely to be identified pre-tx (72%), but 24% (7/29) had ICA diagnosed in the post-tx setting. ICA rupture occurred in 24% (7/29) of PKD KTRs with similar frequency in both pre- and post-transplant (43% [3/7] each).
Conclusion
PKD KTRs can have ICA rupture post-transplantation and thus a post-transplant ICA screening strategy should be investigated in a prospective manner.
