Background

Appropriate blood pressure (BP) goals prevents adverse cardiovascular and renal outcomes in patients with hypertension (HTN). However, several patients do not achieve these goals, even with appropriate therapy. There are several classifications of these patients, such as uncontrolled or resistant HTN, which define the use current and novel therapies, evaluated in recent clinical trials. Currently, there is little data of these HTN profiles on Latin population. Our study aimed to evaluate the prevalence of uncontrolled and resistant HTN in Latin patients.

Methods

Observational, prospective, real-world study. We evaluated Latin patients with previous diagnosis of HTN in control in primary care facilities. We include patients with and without CKD, ASCVD, heart failure, diabetes and obesity. To classify uncontrolled and resistant HTN we used current definitions of 2025 AHA/ACC guidelines, and BP goals according to AHA/ACC (<130/80) and ESC/ESH (<140/90) guidelines. Clinical and laboratory data were assessed. We determined predictors of worse BP control. The study was performed between 01-2025 and 03-2026.

Results

We evaluated 1,319 patients. Age: 68.3±10.3 years. Female: 41%. CKD: 26%. T2D: 18%. ASCVD: 19%. Use of antihypertensive drugs: 93%. Use of 3+ drugs, including thiazide drugs: 23%. Use of monopill therapies: 39%. Systolic blood pressure: 138.2±8.3 mmHg. Concerning the prevalence of uncontrolled HTN, 58% had BP > 130/80 and 39% had BP > 140/90, besides use of antihypertensive drugs. Concerning resistant hypertension, 21% fulfil criteria plus BP >130/80 and 16% fulfil criteria plus BP > 140/90. Evaluating risk factors of uncontrolled and resistant HTN, as compared to those with appropriate BP control, these patients had higher prevalence of CKD and T2D, plus a lower use of monopill therapies.

Conclusion

Our results showed that Latin patients with HTN have a high prevalence of uncontrolled and resistant HTN. These data support the need for appropriate management of these patients, including follow-up and appropriate pharmacological therapies, who are in higher risk of adverse cardiovascular and renal outcomes.
