Background
Malnutrition is highly prevalent among patients receiving haemodialysis. The Global Leadership Initiative on Malnutrition provide a standardised framework for diagnosing malnutrition. Not much is known about the same among MHD patients in India. The objective was to estimate the prevalence and distribution of malnutrition using the GLIM criteria among patients undergoing MHD
Methods
We conducted a cross-sectional study among 374 dialysis patients registered at 4 centres within a large private dialysis network in Andhra Pradesh, India.Study period was from September 2024 to March 2025.Data were collected by dietitians using a semi-structured questionnaire on socio-demographic factors, comorbidities, dialysis factors, anthropometric measurements, nutritional factors and serum albumin. We used GLIM criteria (etiologic criterion of chronic inflammation, phenotypic criteria of reduced muscle mass: MUAC adjusted for BMI ≥ 25kg/m2 different for gender and low BMI: <18.5kg/m2if <70 years or /<20kg/m2 if >70 years) to categorise malnutrition status among dialysis patients. Data analysis was done using SPSS software
Results
Among 374 hemodialysis patients included in the study,153(41%) were categorised as malnourished. All participants fulfilled the etiologic criterion of chronic inflammation due to kidney disease.Regarding the phenotypic criteria, reduced muscle mass was the most common:129 (35%) patients, followed by low BMI:38 (10%) and non-volitional weight loss:12 (9%) patients.Overall,127(34%) participants met at least one phenotypic criterion,25 (7%) met two criteria, and only 1 participant fulfilled all three phenotypic criteria.
Conclusion
Malnutrition was prevalent among two-fifths of MHD patients, with reduced muscle mass being the most common phenotypic abnormality. No socioeconomic, dialysis and clinical factors were significantly different between the groups.
