Introduction
Ethylene glycol (EG) intoxication can result in life threatening metabolic acidosis, kidney failure and dealth. Increased osmolal gap is characteristic when presenting soon after ingestion, but increased anion gap metabolic acidosis (without osmolal gap) is seen late. Managment is multimodal and usually includes hemodialysis. We report a case of a patient who intentionally consumed 3/4 bottle of 99.9% (high purity) ethylene glycol with an initial level of 244 mg/dL.
Case Description
27 year old male with schizoaffective disorder, presented to hospital after ingesting 3/4 of an ethylene glycol bottle. Initial osmolar gap 54 and anion gap 24. Lactate gap present (>17.8 on VBG, discordant from serum 4.1). Also, UA had calcium oxalate crystals. EG level 244 mg/dL. He received Fomepizole and underwent emergent hemodialysis (HD) for 4 hours. 3 hours into session, EG level 101 mg/dL, and down to 75 mg/dL ~6 hours post HD. Next day before 2nd HD, level 70 mg/dL, and 27 post treatment. By next morning, 19 mg/dL.
Discussion
Our report is novel in that it outlines the treatment course and outcome of a high purity EG ingestion (level 244 mg/dL; potentially toxic considered 20 mg/dL). Two hemodialysis sessions were sufficient to lower, and temper the acute kidney injury.
