Background

Middle-molecular weight toxins (MMWT) and inflammatory mediators are involved in increased cardiovascular morbidity and in a high risk of mortality. Thus, we aimed to compare MMWT and inflammatory mediators serum levels of patients after one year of treatment with online hemodiafiltration (OLHDF) versus expanded hemodialysis (HDx).

Methods

A prospective longitudinal study was conducted. Thirty-eight patients with more than 3 months on OLHDF were included. Therefore, 18 patients were included in OLHDF group and 19 patients in HDx. Vitamin B12, endothelin and cytokines (IFN- γ, MCP-1, TNF-a, IL-10, IL-6, IL-1b & IL-8) were measured in blood. B2 microglobulin (B2M), p-cresol and indoxyl sulfate were measured in serum at baseline and after 12 months by commercial ELISA kits according to manufacturer’s instructions. Cytokines levels were quantified by flow cytometry. Either the Mann Whitney U test or Student’s t test was used to assess differences between groups.

Results

After one year of follow-up, we did not observe a significant change in the levels of cytokines but a significant decrease in p cresol, b2 microglobulin was observed from 6 months of follow-up in OL-HDF and HDx group (Table 1). We did not find a significant difference in the comparison of MMWT levels and inflammatory mediators between both groups over the course of a one-year follow-up (Table 2).

Conclusion

The analysis of the outcomes in these patients suggests that the switch from OLHDF to HDx does not represent a significant increase in MMWT and inflammatory mediators and that these toxins maintain stable levels over a one-year of follow-up in both the OLHDF and HDx groups.
