Background
Bloodstream infections (BSI) and catheter dysfunction are major complications in hemodialysis (HD) patients (pts) with central venous catheters (CVC). While the LOCK-IT trial demonstrated reduced CVC-related BSI with taurolidine/heparin (TH) lock solution, real-world effectiveness data remain limited.
Methods
Using administrative data from US Renal Care and Satellite Healthcare, we evaluated HD pts dialyzing via CVC between Mar 2025 and Feb 2026. TH and heparin exposure were modeled as time-varying at the pt-month level. Multivariable Poisson models assessed BSI and alteplase administration; negative binomial regression models assessed missed treatments. To reduce confounding by stable pt characteristics, within-pt fixed effects models were performed among pts contributing both TH- and heparin-exposed months. For missed session analyses, fixed-effects models included pt, calendar month, and facility indicators.
Results
The cohort included 6465 TH-treated pts (31,704 pt-mos) and 11,307 heparin-treated pts (65,864 pt-mos). TH-treated pts were older and had more extensive comorbidity (Table 1). TH use was associated with significantly lower rates of BSI (0.10 versus 0.19 per 1,000 CVC days, adjusted IRR 0.60 95% CI 0.46–0.80, p<0.001), alteplase administration (0.49% versus 0.94% per 100 treatments, adjusted IRR 0.52, 95% CI 0.45–0.61, p<0.001), and missed treatments (IRR 0.93, 95% CI 0.89–0.97), corresponding to relative reductions of 40%, 48%, and 7%, respectively. Within-pt fixed effects analyses were directionally consistent: BSI (IRR 0.41; 95% CI, 0.12–0.93), alteplase administrations (IRR, 0.50; 95% CI, 0.41–0.60) and missed treatments (IRR 0.66; 95% CI. 0.64–0.68).
Conclusion
In a national real-world HD cohort, TH catheter lock solution was associated with significantly lower rates of BSI, alteplase use, and missed dialysis sessions vs heparin. These findings support the effectiveness of TH in routine clinical practice.
