Background

Sodium thiosulfate (STS) may attenuate coronary artery calcification (CAC) and uremic pruritus (UP) in maintenance hemodialysis (MHD) patients, but biomarkers of response remain limited. Extracellular vesicle (EV)-derived miR-122b-3p has been implicated in vascular smooth muscle cell calcification. We investigated whether serum EV-derived miR-122b-3p was associated with STS-related changes in CAC and UP.

Methods

In this open-label, randomized crossover trial, MHD patients were assigned 1:1 to STS or usual care for 8 weeks, followed by crossover after a 2-week washout. Primary outcomes were changes in CAC score (CACs) and pruritus severity by Duo's Pruritus Score. Exploratory outcomes included changes in serum EV-derived miR-122b-3p and its associations with CACs and UP.

Results

Of 93 screened patients, 61 had both CAC and UP, and 37 completed both treatment periods. Compared with control, STS attenuated CAC progression [CACs change: 11.6 (−55.7 to 89.1) vs 59.8 (16.4 to 210.2), P = 0.014] and reduced pruritus severity [−4 (−6 to −2) vs 2 (2 to 2), P < 0.001]. CAC progression was less frequent during STS [20/37 (54.1%) vs 31/37 (83.8%), P = 0.011]. Baseline EV-derived miR-122b-3p correlated with CACs (r = 0.934, P < 0.001). STS decreased miR-122b-3p levels, and miR-122b-3p changes correlated with changes in CACs (r = 0.745, P < 0.001) and pruritus severity (r = 0.427, P = 0.009).

Conclusion

STS attenuated CAC progression and UP. Serum EV-derived miR-122b-3p may serve as a noninvasive biomarker of CAC burden and STS response.
