Background

PD drop-off remains an important challenge, contributing to transitions from home dialysis and increased healthcare utilization. The CloudCath Remote Monitoring System provides remote monitoring of PD effluent during home PD, facilitating earlier recognition and management of complications. We evaluated PD duration before drop-off among patients using CloudCath compared with standard of care.

Methods

We conducted a retrospective observational analysis across 6 PD clinics within 2 dialysis organizations. CloudCath and comparator cohorts were evaluated over the same organizational-specific periods (March 26, 2025–June 11, 2026, and May 1, 2024–May 30, 2026). All PD drop-offs during these periods were reviewed. We calculated PD duration from PD initiation to documented drop-off. 10 patients who transferred to another dialysis facility were excluded because subsequent PD duration could not be ascertained. We compared PD duration across the CloudCath and comparator groups using the two-sided Wilcoxon rank-sum test.

Results

We evaluated 29 CloudCath and 25 comparator patients. Mean PD duration before drop-off was 41.4 months with CloudCath versus 29.9 months with standard of care. Median PD duration with Cloudcath was 41 months (IQR,29-52) versus 19 months (IQR,9-40) without, (P=0.028).

Conclusion

Remote monitoring of spent dialysate using the CloudCath system was associated with a significantly longer duration on PD before dropoff than non-use. These findings suggest that remote PD effluent monitoring may support PD modality longevity and enable patients to remain on home dilaysis.
